Understand the different types of forms that are available in Caredove

Caredove can offer three types of forms: Service Request forms, Routing forms, and Workflow forms. Each type of form has its own use cases.

Service Request Forms

Service Request Form: These forms are launched from, and submitted to your service listings in Caredove. These forms behave as follows:

  • The form can launch from the Caredove search/listings/booking process, and submits to the same listing.

  • The form starts with address eligibility check and eligibility questions from the service listings.

  • The form ends with a standardized Caredove consent statement

Routing Forms

Routing forms contain an algorithm that is used to determine where a form should be submitted, based on the answers to questions in the form. These forms behave as follows:

  • The form can launch from a Caredove search/listings/booking process, but can submit to a difference service based on the routing algorithm

  • The form starts with address eligibility check and eligibility questions from the service listings. [COMING SOON]

  • The form ends with a standardized Caredove consent statement

Workflow [Coming Soon]:

These forms can add additional form data to a service request AFTER the initial request has been submitted (e.g., registrant fills out a "pre-session" form just before an appointment, or the provider fills out an "outcome" assessment form after the appointment, a "survey" form after the appointment). These forms behave as follows:

  • Providers can launch these from a Caredove referral

  • Registrants may be able to launch these from an invitation or confirmation email

  • Only the registrant is presented with a standardized consent statement Caredove consent statement (which differs from the request/referral consent statements)
