AlayaCare is a comprehensive home and community care case management platform that includes:

  • A configurable Electronic Health Record (EHR)

  • Scheduling, point-of-care documentation, and billing tools

  • Client portals, virtual care, and video conferencing

  • Machine learning–based clinical support and predictive analytics


How Does AlayaCare Integrate with Caredove?

Once your organization adds the AlayaCare integration to your Caredove license, our team will activate it and tailor the integration to your workflow. We’ve partnered with AlayaCare to deliver a flexible, pre-configured integration that’s fast to deploy and easy to configure.

This integration provides multiple referral entry points, making it easier to stream referrals into your intake process and elevate client access. These entry points include:

  • Public referral forms on your organization’s website

  • Embedded map-based service catchment tools (e.g. franchise-finder maps)

  • Listings in community or regional Caredove networks

  • Trusted providers or clinicians referring from other systems or directories

How It Works

  1. Receive referrals in Caredove’s inbox from any configured entry point.

  2. Triage and qualify referrals directly in Caredove. Decide whether to manually send only qualified cases to AlayaCare or automate the handoff for all referrals.

  3. Referral data is mapped into AlayaCare, including standard demographic fields, contact details, address, and appointment data. (See full data mapping below.)

  4. Additional information is stored in AlayaCare under referral summary.


Data Fields Supported

Referral information mapped into AlayaCare includes:

  • Client demographics (e.g., name, DOB, gender, language)

  • Contact info (email, phone, address, health card, etc.)

  • Referral metadata (appointment and request dates)

  • Additional intake form responses and free-form notes

Any fields not directly mapped into AlayaCare are available in the Questionnaire field.

Use Cases & When to Send Referrals

You can configure referral triggers to match your workflow:

  • Send after submission—import once referrals are submitted or added to your Caredove inbox

  • Send on status updates—import when referrals are marked “Ready” or “Received”

  • Send on booking or closure—import when an appointment is booked or a referral is closed
