When a referral has been submitted for a client in Caredove, they receive a confirmation email if an email address has been included in that referral.

Within that confirmation email, they will see a section to provide some additional information. The Get Started button links them to the Pre-Session Form.

The pre-session form has no mandatory fields (outside of Consent) so that registrants can complete as much or as little information as necessary.

When they complete and submit the Pre-Session Form, the provider does not receive an email notification. However, the next time they open the client's referral, presumably to provide the appointment, if they scroll past registrant information they'll see the Questionnaire section.

This section will also be available when you select Download.

Then you can view the Pre-Session results within the downloaded PDF.
