Not all intake appointments are completed the same way. For some services, the first step in the intake process might be a phone call or home visit. Caredove has enabled that customization. 

To start, navigate to Settings in the top menu and select the relevant Calendar.
Look for Appointment Locations. Toggle between Yes and No for the locations listed, or choose to Add Appointment Location.
When you create a new appointment, the locations you selected from the settings will be available to choose from.
